I first came to FormMD after having a Mohs procedure on my forehead and had such a great experience that I later returned to have my upper eyelids done. I decided to do the procedure so I would look a little more refreshed for my daughter’s upcoming wedding, and I am so happy I did. Dr. Kasra Ziai did a beautiful job and took the time to explain everything clearly, which made me feel comfortable and confident every step of the way. My eyes look refreshed but still completely natural—exactly what I was hoping for. The bruising and recovery were very minimal, which made the whole experience even better. The entire staff is amazing—so kind, professional, and welcoming. From the first consultation through recovery, I always felt very well taken care of. I highly recommend this practice and would absolutely trust them again in the future.

My experience with the staff and Dr. Polli Was nothing but professional, friendly, knowledgeable, welcoming. I’m An accountant and financial analyst, which makes me very conservative And speculative. My comfort level, because of Dr.Polli’s experience Was very high. I have referred Friends and I’ve also taken my 15-year-old Daughter to see the aesthetician in their office for acne facial. I came to Form MD Plastic Surgery to see Dr. Ziai after I had a Basel cell carcinoma removed from my face by my dermatologist. I was so relieved to know that I would not have a big ugly scar on my cheek. The experience has been wonderful on all accounts from the wonderful ladies up front at the desk, the waiting area is beautiful and stocked with beverages and treats. Dr. Ziai did a wonderful job closing up the procedure I had done and he was very kind and reassuring. I had a follow up shortly after and saw Dr. Pollei who was equally gracious and helpful with all my questions. I am healing up nicely and even my dermatologist said it looks really good. Form MD is extremely professional and excellent at making patients feel comfortable and well treated. I am very grateful for their care.

I’m one month post-op from my septo/rhinoplasty and I honestly couldn’t be happier with my results. Dr. Pollei and his team are truly the best. I would trust them with anything. From my very first consultation all the way through post-op care, Dr. Pollei made me feel so comfortable and confident in my decision. I never once felt nervous or second-guessed myself. He and his entire team really took their time with me, answered every question, and gave me so much reassurance throughout the whole process. I felt genuinely cared for every step of the way. There’s a calm, reassuring energy there that immediately puts you at ease. Dr. Pollei is truly exceptional. He was always available to answer my questions and never once made me feel rushed.
